Awareness and support of services available to assist you and your loved one, and they can help you navigate questions and issues to the appropriate person or department.
Your current treatment team might consist of:
Primary care doctor, other defined specialists, PA/NPs, nurse, case manager, social worker, dietician, pharmacist, rehab team (PT/OT/ST, etc.), financial counseling, home care agency, community, and more.
Did you know that you can call your insurance company and request to have a case manager assigned to your case so that they can assist you in navigating what your insurance company will cover and how to obtain these services or equipment?
